5K - $118K a year Position Summary Uses practice guidelines and clinical judgment to carry out duties.
Accepts responsibility for individual patient care as well as responsibility for addressing broader health care needs and issues; works with others in these areas.
Receives direction from collaborating physicians for medical management of patient care.
Receives direction from Administration regarding nursing responsibilities and management.
Responsibilities Clinical Component Assessment of health status including, Obtaining a relevant health and medical history, Performing a physical examination based on age and history, Conducting preventive screening procedures based on age and history, Identifying medical and health risks and needs.
Updating and recording changes in health status.
Diagnosis, including, Formulating the appropriate differential diagnosis based on the history, physical examination, and clinical finding.
Identifying needs of the individual, family, or community as a result of the evaluation of the collected data.
Development of a treatment plan, including, Ordering appropriate diagnostic tests, Identifying appropriate pharmacologic agents, Identifying non-pharmacologic interventions, Coordinating appropriate referrals, Developing a patient education plan.
Implementation of the plan, including actions that are:
Consistent with the appropriate plan of care and practice guidelines, Based on scientific principles, theoretical knowledge, and clinical expertise, Individual to the specific situation, Consistent with teaching and learning opportunities.
Appropriate in the following respects:
Accurately conducting and interpreting diagnostic tests, Prescribing pharmacologic agents, Prescribing non-pharmacologic therapies, Providing relevant patient education, Making appropriate referrals to other health professional and community agencies.
Systematic follow up and evaluation of patient status, including, Determining the effectiveness of the plan of care through documentation of patient care outcomes Reassessing and modifying the plan as necessary to achieve medical and health goals.

Responsibilities:
Participates as a team member in the provision of medical and health care, interacting with professional colleagues to provide comprehensive care.
Accurate documentation of client status and care:
Maintains accurate, legible, and confidential records.

Works proactively with collaborating physicians and Administration to analyze and resolve productivity problems.
Clinical procedures relevant to specialty area.
Professional component Establishes and meets annual professional goals.
Continuing education and specialty certification:
Maintains CPR certification.
Obtains and maintains prescriptive privileges as per Indiana state law.
Maintains Allied Health privileges at hospitals, when necessary and appropriate.
Collaborating with peers:
Participates in regular Advanced Practice meetings.
Contributes to ongoing development of NP role at clinic.
Mentors new NPs where appropriate Participates in professional organizations as appropriate.
Health care advocate.
May include public speaking, conducting patient groups, providing special teaching, participating in organizational, local, or national groups, and similar activity.
Perform other duties as assigned.
Physical Demands Requires full range of body motion including handling and lifting patients, manual and finger dexterity and eye-hand coordination.
Requires standing and walking for prolonged periods of time.
Occasionally lifts and carries items weighing up to 40 pounds.
Requires normal visual acuity and hearing.
Requires exposure to communicable disease and bodily fluids.
Requires working under stress in emergency situations or during irregular hours.
Knowledge & Skills Organizational policies, regulations, and care procedures.
Requires a high level of analytical skills necessary to assess the patient and participate in follow-up care planning.
Demonstrates interpersonal skills necessary to effectively interact with patient(s), family of patient(s), physicians, other professionals, and paraprofessionals.
Demonstrates high level of communication skills, both oral and written, necessary for effective communication with patient/family, physicians, and referral sources.
Experience and Education Successful completion of a master, post-master, or doctorate degree from an accredited adult nurse practitioner program.
Current license to practice as a Registered Nurse and an Advanced Practice Nurse in the State of Indiana.
Board certification in applicable specialty.
Current and unrestricted DEA license.

Estimated Salary: $20 to $28 per hour based on qualifications.
